Case Study Template

All case studies should be written in accordance to author guidelines (e.g. APA format with references listed). For an example of an appropriate writing style please see Kerr, C.L. (2008). Case studies should include a title page, case presentation, questions and referenced answers. The suggested word count for case studies is 3,000 words. All case studies should be submitted to the Elsevier Editorial System (EES) at http://ees.elsevier.com/jphc.

Presentation of the case study should include the following:

Case Presentation
Chief Complaint
History of Present Illness
Past Medical History
Past Surgical History
Immunizations
Family History
Personal/Social/Developmental History
Review of Systems
Medications
Physical Exam
Diagnostic Testing
(Include any laboratory or radiologic evaluation which are of importance for diagnosis and or follow-up)

Case Study Questions
Four to five case study questions should appear at the end of the presentation and may be related to diagnosis, treatment, intervention and follow-up. For acute and specialty case studies provide needed follow-up with primary care implications.

Examples of questions are:
What differential diagnoses are you considering for this child?
What diagnostic tests/criteria re you considering ordering?
What is your assessment and plan?
What is the recommended follow-up for this child?

Case Study Answers
Provide detailed and referenced answers for each question chosen. Discussion should include the following:
Differential Diagnoses
Assessment, Treatment and Plan
Recommended Follow-up
